Magnesium: Serum magnesium level in preterm labour

Preterm labour, (PTL) defined as labour after 28 weeks but before 37 completed week of gestation, is an ill omen for our country as the incidence is 5-10% leading to 70-80% of perinatal deaths. According to the study by the Indira Gandhi Institute of Medical Sciences, varied hypomagnesemia was observed in Preterm labour cases (1.47 mg/dl +/- 0.22 S.D.), normal value of serum magnesium was found in normal non-pregnant ladies and slightly low value were observed in pregnant ladies of same gestational age. Age and parity had no significant effect on serum magnesium level in our study. As far as socio-economic study is concerned, it was found to be higher in high socio-economic group and low in lower group. Thus from this study it can be concluded that estimation of serum magnesium in pregnancy may prove to be a valuable tool in predicting preterm onset of labour(5).

Gravidae complaining of muscle cramps had significantly lower serum Mg-levels than women without muscle cramps. The administration of Mg was associated with a significant rise in serum Mg-levels as early as 2 weeks after the initiation of therapy.The Our study indicates that nightly muscle cramps during pregnancy might be a sign of a latent magnesium deficiency which can be influenced by oral magnesium(4).

We found no randomized controlled trials evaluating magnesium for exercise-associated muscle cramps or disease state-associated muscle cramps (for example amyotrophic lateral sclerosis/motor neuron disease)(1).

Vitamin B6, also known as pyridoxine, is a water soluble vitamin found abundantly in green peas, yams, broccoli, asparagus and turnip greens,Peanuts, sunflower seeds, cashews and hazelnuts, meat, fish etc., with functions of amino acid, carbohydrate metabolism, brain health, and liver detoxification, etc.

Although vitamin B6 has expressed various health benefits such as induced a healthy metabolism, improved nerve, liver function and enhanced skin health, eye health, over dose of vitamin B6 may cause toxicity in induction of peripheral neuropathy, despite renal excretion of this water-soluble vitamin.

According to the Nathan S. Kline Institute for Psychiatric Research, in the experiment of anephric rats show that the uremic state, in a mere 3 or 4 days, causes a 5- to 10-fold increase in susceptibility to pyridoxine-induced neuronopathy.

Uremia is a serious medical complication of patients with chronic kidney disease and acute kidney injury.

Therefore, there is a need for caution in prescribing pyridoxine to uremic patients who will probably take the vitamin daily for many years. Large amount intake may only prescribe by medical profession.
Further analysis in the toxicity of over dose vitamin B6, researchers at the above university also conducted a experiment of its toxic effect in ther brain found that dietary protein deficiency may be a culprit in elevation of neurotoxicity of pyridoxine.

In a mere 3 or 7 days of pretreatment with either of two protein-deficient diets were sufficient to accelerate and intensify the clinical neurological signs even in the injection of normal dose of vitamin B6.

Additional examination suggested that the elevated levels of vitamin B6 may be casued by at least in part, by loss of body weight, decreased protein binding in serum and decreased consumption of water and decreased volume of urine, which reduce the urinary losses of the toxicant.

In compared to the levels of toxicity of different types of vitamin B6 derivative, pyridoxine (pyridoxal, pyridoxamine) and the coenzyme (pyridoxal 5-phosphate) did not cause clinical signs or lesions similar to those produced by pyridoxine even when injected in maximum tolerated doses.

More interestingly, in an examined the sequence of nervous system abnormalities that resulted when rats given excess amounts of vitamin B6 (pyridoxine of 1,200 or 600 mg/kg/d) for 6 to 10 days researchers suggested that high dose of vitamin B6 are sensitively affect the large diameter neurons with long processes and large cytoplasmic volumes in compared to application of smaller doses (300 to 150 mg/kg/d) for up to 12 weeks with lesser effects on DRG neurons, but produced a neuropathy with axonal nerve fiber atrophy and degeneration

Dorsal root ganglion (DRG) neurons is a cluster of nerve cell bodies in a dorsal root of a spinal nerve, containing the cell bodies of sensory neurons.

The effect of vitamin B6 are also depending types of animal model used in the experiment as guinea pigs given 1,800 mg/kg/d developed sensory neuronopathy, whereas mice given similar or higher doses did not have neuropathologic abnormalities.

Therefore, multiple factors must be taken into consideration, such as rate of administration, differential neuronal vulnerability, and species susceptibility before one can make the final expression of pyridoxine neurotoxicity.

Taking altogether, vitamin B6 may have many health benefits but intake of large dose as vitamin supplement can only be prescibed by medical professional to prevent neurotoxicity.

Although vitamin B6 and its derivative have shown a strong impact in treatment of all groups of epilepsy, intake of antiepileptic drugs do not influence to the expression of plasma levels of vitamin B6, according to the University Hospital Zurich, in the study of analyzed vitamin B6 blood levels in 400 serial patients with epilepsy, AED-treated (n=385), untreated (n=15) and healthy controls (n=233).

But further differentiation of the interaction of vitamin B6 in patient with epilepsy underdone treatment of antiepileptic drugs (AEDs) including phenytoin or carbamazepine) and non-inducing AED (levetiracetam, lamotrigine, or topiramate), researcher found that
1. Contrast to general belief that intake of antiepileptic drugs did not lower either levels of folate nor vitamin B12, observed by blood test.
2. Vitamin B6 deficiency was found in 16/33 patients (48%) in compared to 1/11 controls (9%; p=0.031)
3. Patients in antiepileptic drugs group showed a significant in vitamin B6 deficiency of only 7 patients (21%) .after switching to non inducers.

After taking into account of con founders, Dr. Mintzer S, the lead author said, "The incidence of deficiency was similar regardless of which inducing or non-inducing AED was being taken" and "treatment with inducing AEDs commonly causes pyridoxine deficiency, often severe".

Intrusively, the study also address that other health conditions in patients with epilepsy also must be taken into consideration before injection of older AED as the medicine may induced neuro damage and chronic heath difficulties.

More importantly, in the study to determine the means of determination of activity of EGOT in 30 and of serum pyridoxal phosphate in 27 epileptic children under antiepileptic treatment with various antiepileptic drugs and with hydantoin and succinimide alone, 25 healthy infants and children were controls, researchers indicated that

Therefore, vitamin B6 testing in patients treated with hydantoin and succinimide may be necessary to prevent risk of reduced vitamin b6 metabolism cause of toxic effects in several neurological pathologies.

Taking altogether, injection of antiepileptic drugs  may have a complicative effect in reduced vitamin B6 metabolism in induction of neuro damage and chronic heath difficulties.

The effect of vitamin B6 in treatment of pyridoxine-dependent epilepsy was under microscopic investigations extensively through many aspects, particularly in vitamin B6 application.

In inborn errors of autosomal recessive inheritance that lead to vitamin B6-dependent seizures, after taking account of other con founders, researchers suggested that a vitamin B6 withdrawal for diagnose should also be considered as an therapy for resistant neonatal seizures as early consideration of a vitamin B6 trial over 3 consecutive days is crucial in order to prevent irreversible brain damage, instead of only for patients with a clear vitamin B6 response.

Neonatal seizures, a medical condition of occurrence of seizures due to neurologic dysfunction in the neonatal period.

Pyridoxine, an the active form of vitamin B6 would be effective commonly in treatment of vitamin B6-dependent seizures, but pyridoxine showed no signs of potency in patients with neonatal seizures due to PNPO deficiency.

The use of PLP in patients with resistant neonatal seizures should be taken with care with resuscitation equipment at hand during a first pyridoxine/PLP administration, as severe apnea has been occurred in patients, researcher continued.

Unfortunately, due to PLP is unlicensed within Europe and North America, pyridoxine is widely used as the first line drug, therefore for patient do not response to the treatment of PLP, some researchers suggested that PLP injection may be necessary and should be followed by a trial, especially in neonates.

Furthermore, caregivers, including parents with children with epilepsy should be inform about the lifelong dependency and recurrence risks in forthcoming pregnancies.

Further analysis of  the resistant conventional antiepileptic medications of early-onset epilepsy in children with neonates caused by hypophosphatasia and congenital rickets and in compared to patients of hyperprolinemia type 2, antiquitin deficiency, and pyridoxine phosphate oxidase deficiency of epilepsy, also indicated that although many patients with epilepsy are response to treatment of vitamin B6 injection but PLP treatment was more effective than pyridoxine, through some unknown mechanisms.

Dr. Wang HS, the lead author said, "Since PLP is as inexpensive as pyridoxine, we suggest replacing PLP for pyridoxine when treating children with epilepsy".

Additionally, Pyridoxine-dependent epilepsy (PDE) may also be caused by genetic mutation of certain genes, such as c.1597_1597delG mutations in ALDH7A1 gene.

Observation of patients data in 3 groups with complete seizure control with pyridoxine and normal development (group 1), complete seizure control with pyridoxine and development delay (group 2), and patients with persistent seizures despite pyridoxine treatment and with development delay (group 3) researchers at the Çukurova University Faculty of Medicine found that early diagnosis and treatment of these patients is vital. Patients with persistent seizure despite pyridoxine treatment and with development delay should be diagnosed in condition of any neonate with signs of encephalopathy and refractory seizures.

Taken together, if patients with epilepsy do not response to treatment of application of vitamin B6, PLP injection may be considered but substantial care must be taken  to reduce risk of complications of severe apnea.

All About Vitamins: Vitamin B6 in Reduced Risk and Treatment of non-small cell lung cancer (NSCLC)

Vitamin B6 may have a profound effect in reduced non-small cell lung cancer (NSCLC), some scietists suggested.

Vitamin B6, also known as pyridoxine, is a water soluble vitamin found abundantly in green peas, yams, broccoli, asparagus and turnip greens,Peanuts, sunflower seeds, cashews and hazelnuts, meat, fish etc., with functions of amino acid, carbohydrate metabolism, brain health, and liver detoxification, etc.

Non-small cell lung cancer is the most common type of lung cancer with characteristic to grow and spread methodically and predictably.

In the study to examine the clinical outcomes of the sodium cantharidinate/vitamin B6/GP regimen in the treatment of middle-late stage Non-small-cell lung carcinoma (NSCLC) with
86 cases of NSCLC selected and randomized into two groups; 45 cases in the treatment group were given 30 ml cantharidinate/vitamin B6+ GP regimen, for four 21-day cycles and 41 cases in the control group were subject to regular GP regimen, for four 21-day cycles, at the end of experiment, researchers found
1. Treatment group showed a significant improvement with effectiveness rate of 57.8% in compared to 36.6% in the control group,
2. Patients in group demonstrated an improvement of quality of life
3. Treatment group also displayed a lower toxic/adverse effects in compared with that in the control group.

After taking into account of other risk factors, Dr. Wang B, the lead author said, "odium cantharidinate/vitamin B6/GP regimen had fair effectiveness and synergistically improved the clinical outcomes. It lowered the toxic/adverse effects and its application is worth further investigation and promotion".

Other, in the study to examine the effect vitamin B6 for treatment of non-small cell lung cancer, after observation of vitamin B6 metabolism as a central regulator of cisplatin responses in vitro and in vivo, researchers at the INSERM showed that
1. Vitamin B6 sensitized lung cancer cells to apoptosis induction by distinct types of physical and chemical stress.
2. Pyridoxal kinase (PDXK), the enzyme played an important role in generated the bioactive form of vitamin B6 and deficiency of PDXK expression levels were associated with poor disease outcome in two independent cohorts of patients with NSCLC.

The result of the study demonstrated the important role of pyridoxal kinase (PDXK) in exhibited vitamin B6 in reduced risk and treatment of NSCLC.
In fact, most patients with NSCLC are found to correlation with low levels of pyridoxal kinase (PDXK).

Taking altogether, vitamin B6 may be considered as a secondary and therapeutic treatment of NSCLC.

Vitamin B6 may have a intrinsic effect in reduced symptoms of depression, some scientists suggested.

Vitamin B6, also known as pyridoxine, is a water soluble vitamin found abundantly in green peas, yams, broccoli, asparagus and turnip greens,Peanuts, sunflower seeds, cashews and hazelnuts, meat, fish etc., with functions of amino acid, carbohydrate metabolism, brain health, and liver detoxification, etc.

Depression is a normal response as part of our daily lives such as the loss of s job, the death of a love one, and illness. Over 30 million Americans suffer from depression and the amount is increasing in an alarming rate.

In the study to investigate whether different intake levels of folate, vitamin B6 and B12 were associated with a 3-year depression incidence among generally healthy, community-dwelling older men and women conducted by the joint study lead by the St Francis Xavier University, researchers showed that
1. Vitamin B6 intake from foods were associated to 43% less likely to become depressed when adjusting for demographic and health factors in women.
2. Risk of depression also decreased in men who took dietary vitamin B6 during the study.


The results indicated that dietary vitamin B6 may be the choice for both men and women to reduced risk of depression.

Other in the study to investigate whether adolescent pregnant women were on greater risk of nutritional deficits in stress, and depression than their adult counterparts, and these risk factors associated to adverse pregnancy outcomes, researchers at the joint study lead by the Columbia University Medical Center, suggested that vitamin B6 was one of many micro nutrients found in deficient status in these group of women.

After taking into account of other risk factors, Dr. Singh A, the lead author postulated, "mood and dietary factors are associated and should be considered together for health interventions during adolescent pregnancy for the young woman and her future child".

Taking altogether, vitamin B6 may be considered as an adjunct supplement with standard therapy for reducing risk of depression in general population.

All About Vitamins: Vitamin B6 in Reduced Psychological symptoms of Schizophrenia

Vitamin B6 may have a potential effect in reduced psychological symptoms in chronic schizophrenic and schizoaffective patients, some researchers suggested.

Vitamin B6, also known as pyridoxine, is a water soluble vitamin found abundantly in green peas, yams, broccoli, asparagus and turnip greens,Peanuts, sunflower seeds, cashews and hazelnuts, meat, fish etc., with functions of amino acid, carbohydrate metabolism, brain health, and liver detoxification, etc.

Schizophrenia is a severe mental disorder characterized with inability to interpret reality normally and Schizoaffective disorder is also a chronic mental health condition characterized primarily by symptoms of schizophrenia.

In a study to examine the effects of vitamin B6 reduced psychotic symptoms of psychopharmacologic treatment of patients suffering from schizophrenia or organic mental disorder, of spanning 9 weeks with all patients with stable psychopathology for at least 1 month before entry and randomly received placebo or vitamin B6, starting at 100 mg/day in the first week and increasing to 400 mg/day in the fourth week by 100-mg increments each week, researchers found that there are no differences between vitamin B6- and placebo-treated patients in amelioration of mental state of attendees.

Dr. Lerner V, after taking into account of other confounders, said, "Further studies with larger populations and shorter duration of illness are needed to clarify the question of the possible efficacy of vitamin B6 in treatment of psychotic symptoms in schizophrenia".

Other researchers in the confirmation of vitamin B6 great progress in the treatment of positive symptoms in schizophrenic patients and movement disorders induced by neuroleptics,at the Ben Gurion University of the Negev, launched a a double-blind crossover controlled during 9 weeks with patient treated with up to 400 mg/day vitamin B6 versus placebo, after reviewing the result taken from Positive and Negative Symptoms Scale (PANSS), Extrapyramidal Symptoms Rating Scale (ESRS) and a blood sample of level pyridoxal showed that vitamin B6 treated group demostrated a significant improvement in tardive dyskinesia and parkinsonian symptoms. but failed to express therapeutic effect of psychotic symptoms from vitamin B6 added to antipsychotic agents.

The contradictory findings suggested that further studies with larger samples and higher doses of vitamin B6 are necessary before clarifying the possibility of a positive therapeutic effect of vitamin B6 as an augmentation agent for the treatment of psychotic symptoms.
